BIRTH CERTIFICATE

Birth Certificates are issued for those births that occurred in Cream Ridge, Upper Freehold Township only.  The fee for a birth certificate is $15.00.  Identification must accompany a request for a birth certificate and the certificate can only be issued to a parent or the individual himself.

DEATH CERTIFICATE

Death Certificates are issued for those whose death occurred in Cream Ridge, Upper Freehold Township only.  The fee for a death certificate is $15.00.  Identification must accompany a request for a death certificate.  Only a direct family member, i.e. spouse, sibling, or child can request a certified copy.  Proof of relationship must accompany the request.  For example, a child would need to provide a full copy of their birth certificate showing their parents full names on the certificate.  Checks are to be made payable to Upper Freehold Township. MARRIAGE CERTIFICATE

Marriage Certificates are issued for those whose marriage occurred in Cream Ridge, Upper Freehold Township only.  The fee for a marriage certificate is $15.00.  Identification must accompany a request for a marriage certificate.  Checks are to be made payable to Upper Freehold Township.  Please call 609-758-7738 ext 210 or 224 so that we can make sure we have your record on file. 

OBTAINING A MARRIAGE LICENSE IN UPPER FREEHOLD TOWNSHIP

WHO SHOULD APPLY IN UPPER FREEHOLD
            Couples (at least one partner) who resides in Upper Freehold Township.  If both persons are out of NJ then in the
            town where the ceremony is taking place

WHERE TO APPLY-
            You must call to make an appointment at 609-758-7738 ext 210 or 224. 
            Clerks Office, 314 Route 539, Cream Ridge
            Monday thru Thursday from 9 - 4 excluding holidays.
            No application will be taken after 4 p.m.

PERSONS REQUIRED TO BE PRESENT AT TIME OF APPLICATION
            Bride, Groom & 1 Witness, who is 18 years or older, knows the couple & speaks English.

ITEMS REQUIRED AT TIME OF APPLICATION - ALL DOCUMENTS MUST BE IN ENGLISH 
            1.  Driver's License of Birth Certificate or passport
            2.  Proof of residency such as driver's license, lease, tax return, etc.
            3.  Name & birth place (country or state) of both sets of parents (maiden names of mothers)      
            4.  Name, address & phone number of the person performing the marriage ceremony
            5.  Name of the place, and city where the ceremony will be held
            6.  $28.00 payable in exact cash, check or money order
            7.  If married before, death certificate or divorce papers, whichever applies 
            8.  Social Security Cards or Social Security Number

WHEN TO COME IN AND WAITING PERIOD, AFTER FILLING OUT THE APPLICATION
            The recommendation is to come in at least two weeks before the ceremony.  There is a mandated 72-hour
            waiting period from the time of application.  (Remember, we are closed on Fridays, weekends and Holidays,
            which means you must apply by Monday in order to pick up the license on Thursday, for Friday, Saturday
            or Sunday marriage).  The marriage license is good for 30 days from the date of pick up.

REMARRIAGE LICENSE
            Bring your marriage license.  No waiting period required.  Please call if you have any questions.

MINORS - (UNDER 18 YEARS OLD)
            Contact Vital Statistics for specific information.

FILING & CERTIFIED COPIES OF THE MARRIAGE LICENSE -
            The marriage license will be recorded in the town where the marriage ceremony took place, and the certified
            copies can be obtained from them also.
